Level of Care Assessments
We assist Medicaid beneficiaries with mental illnesses, disabilities and long-term care by reviewing their eligibility for state wavier programs to determine if additional services are needed. The level of care is determined by aligning quality metrics and using standardized assessments. In addition, we also review previous level of care determinations to ensure accuracy of decisions and current services provided.
Preadmission Screening and Resident Review (PASRR)
We provide PASRR services that deliver reliable, timely, and clinically-valid recommendations and determinations that protect the interests of the individual, while ensuring state compliance with federal statutes. Services include: Level I screening, Level II evaluation, determination and notification processing, comprehensive PASRR IT system, and extensive daily and monthly reporting.
Behavioral Health Needs Assessments
Acentra Health facilitates the administration of Behavioral Health Needs Assessments for individuals with Intellectual and Developmental Disorders (I/DD). Our program engages individuals and their support network to ensure a well-informed assessment of the individual’s needs.
